  DISABILITIES Support Services (DSS)

SEMI-INDEPENDENT LIVING PROGRAM

This program is usually referred to as the SIL program.  When an individual chooses this service, it is explicitly designed to meet that one person’s needs. Please remember this program is set up to do with and teach as much independence as each individual can learn, manage and maintain.

The SIL program includes the following services:

        Daily Living Skills

o   Cleaning, Organizational, Meal Planning, Grocery Shopping, and budgeting skills.

        Employment and Employment Prep

o   Resume writing, interview prepping and role-playing, Building Community Connections

o   Job Placement, Workplace advocacy, Needs and Support assessment, Building Workplace Relationships, Coaching

        Community Access

o   Building positive community-based experiences. Accessing community Opportunities, making valuable interactions and contributions within a community

        24-Hour Group Home

o   In-Home Living with 24-hour support staff

ALBERTA BRAIN INJURY SERVICES

Focus provides services for survivors of Brain Injury.  Goals and Objectives are set out with Canadian Mental Health Services and Focus staff to deliver quality services to assist survivors in meeting their goals and objectives positively.

FEE FOR SERVICE

Focus offers qualified services for a fee which allows the families or individuals participating in this program to design a program specifically to meet their tailored needs.  This is usually a short-term program.

Family and Child Support Services (FCSS)

FAMILY SUPPORT

This program works in partnership with the Government of Saskatchewan Ministry of Social Services to provide for families in crisis. The program works with parents equipping them with the resources and tools that they need to manage daily living challenges such as budgeting, planning and household upkeep.  The program also works with parents to develop stronger, successful communication and parenting practices.  Services are also offered to support parents who deal with addictions, mental health and social challenges.

YOUTH SUPPORT

The Youth Support program offers support for youth who are at risk or are experiencing trauma.  The program provides the resource and training for youth and children to develop health options when dealing with such things as addiction, mental health, grief, abuse.  The program offers a supportive role, and the caseworkers act as advocates for the persons in their care.

FEE FOR SERVICE

FOCUS offers qualified services for a fee designed to help individuals who are at risk or experiencing trauma receive the support they need.  FOCUS works in partnership with other organizations to help clients manage life challenges such as walking through grief, mental health, suicide, abuse, depression and addictions.  This program looks at innovative and practical ways to train, support and equip a person or family so they are empowered to manage their situation successfully.

FOCUS Transportation Services (FTS)

Fee for service

The FOCUS Transportation Service offers a fee-for-service to private residents who are needing transportation to and from a medical appointment in the Vermilion and area, as well as to larger city centers. Oftentimes it is difficult to find transportation to appointments in city centers, these challenges compound when considering the challenges of finding parking. FTS offers the peace of mind that you will be dropped off at your appointment in a timely manner and you will be picked up by the same vehicle when your appointment is complete. Fee-for-service offers curbside drop-off to private residences, housing facilities and medical facilities. FTS is committed to non-emergent medical appointment transportation.

Contract services

The FOCUS Transportation Service offers contract and fee-for-service agreements for organizations and facilities that need ongoing transportation of their residents and clients to diagnostic, medical, specialist and follow-up appointments. The FTS service does not replace the need for medical transportation from Ambulance services. Ambulance services are reserved for emergent transport and transport requiring medical treatment while en route to their destination. FTS is a great alternative for medical imaging appointments, follow-up appointments and specialist appointments. Furthermore, FTS can be utilized to return clients to their residences after hospital stays.

Adopt a ride program

The Adopt-a-Ride program is a great way to put back into your community. By donating the cost of a trip to a medical appointment you will be making it possible for persons who otherwise would not be able to afford the cost of getting to their medical appointments. The Adopt-a-Ride program sets aside donated funds for persons from vulnerable sectors, thus ensuring their medical transport needs are met.
